An email address must be connected to your account in order to reset your username. For assistance, please contact the HIM department at (626) 814-2440 or send an email to Emanate.Health.Patient.Portal@emanatehealth.org. The department is open Monday thru Friday, 8am to 4pm PST, excluding holidays.

This will reset both your username and your password.
